WASHINGTON (Reuters) - An analysis by human rights lawyers has found numerous flaws in U.S. military investigations into deaths of prisoners in U.S. custody in Iraq and Afghanistan, including failure to interview key witnesses or collect and preserve evidence usable by prosecutors.
