main ones i can think of would be bad intelligence during the pacifc campaighn, and letting general macarthur command would be the other one.

I would say for not knowing enough about what Jappanese thought of POWs and for not believing that the Germans had a enough resources to lanch a Counter attack in the winter months of 1944.
